A 45-year-old male client who has been diagnosed with high blood pressure (BP) 'hypertension'. He smokes a pack of cigarettes daily and drinks alcohol occasionally. He lives at home with his wife and a teenage son.  He works full time and goes to the gym on a sporadic basis. This client has had elevated BP on multiple occasions one month apart. He has seen his doctor once and was prescribed blood pressure lowering pills. 

Where we come in as your RN Coach? 

We will gather information from you about your health concerns and find out your overall health goals and preferences. From there, we will partner with you and your specialists to create a comprehensive holistic wellness care plan that will suit your needs.  We will perform our nursing assessments and assist in monitoring your medication effects; document and log your daily activities; assess your dietary requirements; and connect you with the right resources and specialists, on a as needed basis. 

What is the advantage of having a Nurse Coach? 

The comprehensive health package, documentation, and resources we create with you can be presented to your doctor or your other specialists in order for them to gain a better detailed understanding the of your health care needs. The goal is to prevent complications, reduce your hospital admissions, and introduce continuity of care through the use of comprehensive care planning, nursing assessment, coaching, and documentation.

Seniors wanting to live independently seeking resources for activities of daily living with in home supports, assisted living, and long term care eligibility/options.

We will work with you to determine the best possible housing options in your area based on your care needs.

In terms of home support services, no matter what your health needs are, having a nurse support your wellness goals will keep you on track to meeting them. We work together to connect you with the appropriate services you may need.

A 74-year-old female client living alone in the community has had several falls and hospital admissions. After recovering from a recent fall at home, she made a decision to downsize and sell her home. She wants to get options presented to her about Assisted Living; Supportive Housing; or Long Term Care where she can get either 24 hour care or alternatively some supports with activities of daily living (if need be). 

Where we come in as an RN Consultant?

We can assist in finding you the home that best suit your health needs from a mental and functional perspective. A home with good reputation in the community. We will provide you with education on costs of private versus subsidized homes. As well as look for home care agencies that can best support your daily activities with good reviews. Our services of finding you the right seniors home and find you reputable care givers can alleviate headaches of not knowing where to start, if you are to find a place on your own. We are here to help guide you to finding quality, reasonable, affordable housing options.

We also provide 'Monitor' services in a Representation Agreement. The monitor role as a problem solving role: if a third party had a concern about an action of a representative, the monitor is the person on the team who would look into it.  The law states that the monitor needs to speak to the adult, the representative and anyone else appointed in the Agreement such as other representatives and alternate(s). The idea is to see if there is a problem – most often it is an issue of miscommunication – and if there is, get things back on track.

An 85-year-old male client admitted to the hospital and was not capable of informed consent. The daughter indicated that the medical team wanted to perform surgery. The daughter said No to surgery. The client made a Representation Agreement ahead of time,when he was capable of understanding and naming his daughter as the Representative.

Where do our services as a 'Monitor' come in?

In this case, the client could name us as a 'Monitor' under the Representation Agreement, while capable, to ensure that his Representative will always be acting on his best interest when he becomes incapable and can no longer provide consent to treatment options.
